Instructions to complete the Course of Study Form
1. Fill in the school year.
2. Include the child's and parent's full name. This is very important if there are different last names between
children and parent.
3. Include the grade level for which your child is in.
4. Under Subject please indicate the exact subject. For example: enter Algebra I, Algebra II, Chemistry, Biology
instead of the generic terms Math or Science.
5. Include the name of book or program under Book, Activity section. If you are taking a class at EIE then you can
just mention the class and teacher.
6. Make any comments you wish in the Comments Section
7. You do not have to fill in every line. Only include areas that pertain to your student.
8. Mail or bring completed forms to our Resource Center.
